What is skin rejuvenation and resurfacing? Sun, skin disorders, aging, and even heredity can all contribute to skin irregularities on the face and elsewhere on the body. These include irregularities such as changes in texture, wrinkles and acne scars, pigmentation issues, sun damage, freckles, melasma, broken blood vessels, or skin redness tone due to sun damage or a loss of pigmentation (hypopigmentation). As a result of aging, loss of tone, firmness, and overall healthy glow that is found in youthful skin occurs. Skin rejuvenation and resurfacing can be achieved in a number of ways, ranging from laser, light and other energy-based treatments, to chemical peels and other non-ablative methods. Combining skin rejuvenation and resurfacing treatments with fillers or neurotoxins along with medically based skincare products is often recommended to patients wanting to restore their youthful appearance without the downtime or risks of surgery.

Dermaplaning

This chemical and acid-free skin rejuvenation technique is perfect as a prep prior to a peel or other skin resurfacing treatment. Removing dead surface cells and villus downy hair from the face help introduce products into the skin that would normally sit on the surface. Dermaplaning is recommended once every four weeks or sooner for patients that tolerate the technique and like the way the skin looks and feels post-treatment.

AHA/BHA peels

Alpha hydroxy or beta hydroxy peels are a great way to keep skin looking fresh and encouraging new cell turnover, resulting in collagen stimulation and remodeling. Beta hydroxy acids such as salicylic are often times recommended for a patient who is more acne prone. Alpha hydroxy acids, such as lactic acid are oftentimes applied to dry or dehydrated skin types. Patients being seen on a monthly basis may start with a beta hydroxy acid peel and later switched over to an alpha hydroxy peel to ensure that overtreatment and skin inflammation does not occur.

Chemical Peels

A chemical peel is a synthetic type of acid used with the intent of penetrating quite a bit deeper into the skin. Chemical peels are often replaced with other modalities such as laser technology, micro needling, and other non-ablative options but can be an effective alternative option for many patients. The peels are associated with some downtime and should be discussed prior to use.  They are an excellent choice for hyperpigmentation. TCA and Obagi Blue peels are often recommended for patients wanting a true skin transformation in a short period of time.

Microneedling

EssenceMD offers the Gold standard in microneedling with the FDA approved SkinPen microneedling device. The SkinPen causes controlled micro-injuries that stimulate the body’s natural wound healing process. The micro-injuries trigger the release of cytokines and growth factors that lead to remodeling of collagen and elastin. It’s an excellent treatment choice for patients with acne, acne scarring, deeper lines and wrinkles, large pores, hyperpigmentation and photo damage. Downtime is 24 hours post treatment without makeup and moderate redness subsiding within 72 hrs.

There are three phases to the wound repair process:

Phase I: Inflammation. Piercing the skin triggers your immune system to disinfect the wounds, remove debris, increase blood flow and begin to create new tissue.

Phase II: Proliferation. The wound is re-built with new granulation cells, as well as a temporary (type III) collagen and elastin, which are part of the extracellular matrix. Additionally, a new network of blood vessels develops.

Phase III: Remodeling. The wound has been replaced with new dermal tissues and blood vessels. The type III collagen in the newly formed tissue is replaced with stronger type I collagen, which can contract to create a tightening or “shrink wrap” effect on the skin. Patients can expect immediate results from the first treatment but optimal results are often seen following third microneedling treatment. Individual treatments are spaced four weeks apart. Patients may return for dermaplaning in between at their follow up visit if recommended.

Cutera Laser Genesis

Using innovative Nd:YAG laser technology this non-ablative, no downtime laser treatment stimulates collagen re-growth and encourages cell production promoting vibrant healthy skin. Laser Genesis works by warming the dermis and stimulating collagen growth. The heat combined with the light of the laser also targets pigmented clusters beneath the skin restoring the skin to a healthy youthful state resulting in a more even skin color, firmer tone, and finer texture.  It is an excellent treatment to reduce pore size and fine lines. Treatments are often done in a series and followed by routine maintenance to keep the skin vibrant and smooth.

Fillers

Dermal fillers are products that are made from substances found naturally in the body or synthetically made to mimic natural substances or stimulate collagen production. At EssenceMD these products are used in various procedures including lip shaping and enhancement, cheek augmentation, filling of deep lines of the face and neck, hand rejuvenation, buttock lift, and enhancement, and under the eye circles.  Another popular procedure is the “liquid facelift” using a combination of multiple fillers and PDO threads. EssenceMD has a variety of filler brands including Juvederm, Boletero, Restylane, Radiesse, and Sculptra that allows for a tailored individual approach to filler selection and outcome.

Neurotoxin

Neuromodulators such as Botox, Xeomin, and Dysport can be used to soften dynamic lines of the face and are one of the most popular injection treatments. We like to say at EssenceMD that “almost everyone looks great after a little Botox”. In addition to their use on the face, they can also be used to reduce the size of prominent neck bands, painful TMJ, and excessive underarm or hand sweating. Injection of Botox and other neuromodulators is simple with essentially no discomfort and can be done immediately after an initial consultation.  The effects are usually seen in about one week and can last 3-6 months.

Kybella

Kybella is an FDA approved medication for the reduction of under the chin fat. Kybella is a component of the bile that naturally occurs in the body known as deoxycholic acid and works to degrade fat cells. Treatment typically involves several injections in 1-4 separate sessions spread several weeks apart.  Kybella is also commonly used off-label in other areas of the body to reduce troublesome pockets of fat. At EssenceMD we will commonly combine Kybella with Trusculpt 3D for under the chin fat that to enhance fat reduction and provide some skin tightening.

Cutera Limelight Laser (IPL)

The LimeLight Facial can treat all regions of the skin on the face, neck, décolletage, and hands. It ideally treats the skin’s appearance of redness – especially the tiny veins or brown spots that are usually hard to treat, as well as sun damage, freckles, rosacea, and broken capillaries. In order to break down the pigmented area of the skin, the Intense Pulsed Light (IPL) uses numerous wavelengths of light to treat the damaged area that the skin then absorbs. The light energy then breaks down the affected area without affecting the healthy surrounding skin surface. The body then removes the damaged skin, leaving a well-rejuvenated complexion that is smoother and clear. The Limelight facial is an excellent treatment for treating sun-damaged skin. Treatments are quick and easy with minimal discomfort and are usually done in a series a few weeks apart.

Latisse

Latisse is an FDA approved treatment for improved eyelash growth. After 16 weeks patients should experience fuller, darker, and longer eyelashes. Latisse is not for everyone and requires a prescription. Call today to schedule a free consultation!

Microblading

Microblading is a semi-permanent tattooing technique that creates hairlike strokes to fill in sparse or thinning brows and has become one of the most popular aesthetic treatments. The practice aims to give you an effortless full brow ― no need for expensive brow pencils or gels. The best part about the technique is that there is no downtime. Your new set of brows will be ready for a selfie immediately after the process.  Most patients will require a brief touch up in about one month. Unlike eyebrow extensions, which you have to be careful about maintaining, microblading is low maintenance. After healing, you can rub them and enjoy swimming. No special care is required, except for a brief touch-up once a year.

PDO Threads

PDO threads are the latest revolutionary treatment for skin tightening and lifting. The threads are made of PDO (polydioxanone) material which is the same self-dissolving substance used in surgical sutures. When inserted into the skin, the PDO threads act as a “scaffold” for the skin which helps to “hold” the skin against the effects of gravity and aging. The “mesh” of threads will also shape the face by lifting and supporting sagging structures. PDO sutures will continuously stimulate collagen synthesis under the skin. These thread sutures are absorbable in 4-6 months leaving nothing behind but the skin structure created which typically continues to give results for up to 2 years.

The advantages of PDO threads is that they are easy to place with no downtime or surgery. They can be put in using topical anesthesia but many treatments are done comfortably without topical numbing.  A typical procedure will be less than 30 minutes and the results are immediate and long-lasting. They are often used in combination with fillers and Botox. PDO threads are commonly used for lifting the brows, face, and neck as well as several other areas on the body.

Profractional Laser

Pearl Fractional laser resurfacing is an effective treatment for significant sun damaged skin, deep lines and wrinkles, and textural issues on the face & neck. Pearl Fractional is a fractionated laser that can take years off your appearance by improving imperfections due to sun exposure, genetics, and the natural aging process. Results can be obtained in one or two treatments.

During the procedure, the profractional laser uses heat to create tiny holes, the size of a pore, deep in the skin. This “fractional” treatment selectively removes columns of sun-damaged tissue leaving the surrounding skin intact which stimulates the growth of healthy, new skin, full of fresh collagen, plumping the skin from below.  A typical profractional procedure takes about 45-60 minutes. Minimization of discomfort is achieved using topical anesthetic cream prior to treatment as well as using chilled air during treatment.

The healing time is shorter than other ablative lasers but is still up to a week of social downtime. Most patients are completely healed by 5-7 days. Improvement in skin texture and appearance continues over weeks and months as your skin continues to produce new collagen in the treated area, producing longer beneficial changes associated with a healthy, youthful appearance. Many patients have a Pearl Fractional treatment in combination with other cosmetic therapies, such as intense pulsed light (IPL), Laser Genesis, Botox, and soft tissue fillers.

Laser hair removal is one of the most popular cosmetic procedures. Laser hair treatment involves light energy delivered to the skin that destroys the hair follicle. It can be performed on all areas of the body and is associated with minimal discomfort. Treatments are usually 30-45 mins and need to be performed in a series several weeks apart.  Maintenance treatments are also sometimes needed for optimal results.
